Restoring files that have been deleted

We're currently experiencing a recurring issue with a specific user of our Mahara setup (1.3.3) where a handful of his files that were added to views have been deleted from the file system.

Now we're investigating why these deletions are happening, but for 3 out of the 5 files, we restored them to their original locations on the file system and they began working again without issue. However, two of the files, both images, were restored to their original locations and still come up with broken image links and "Access Denied" when anyone tries to access them.

I haven't had a chance to look at the database entries for these files yet (security policy at my work says only DBAs can see production databases) but I was wondering if anyone had encountered anything similar to this.

For reference, the files had the ids 336 and 337 and were restored to ../maharadata/artefact/file/originals/80 and ../maharadata/artefact/file/originals/81.
